Please note that forms for late and missed breaks are presently available . These forms are to be used for taking notes during delay / cancellation.
Forms are established depending on your work shift, 8h and 10h. Members working 4h shifts, can use the 8h pm form . Please make sure to indicate 4h instead of 8am on sheet .
Online forms can be found on AC-SO.org in the documentation section. Forms are available in the Break room at transborder and in the water and glove room at ID.
Insurance forms and reports can also be found there. If any member lacks copy's, please report it to a delegate who can refill them within 5 minutes
